Fees vary depending on the
complexity of the service you receive. Special tests or
procedures generally add to the cost of a routine exam.
The patient is responsible
for co-pays or other fees required by your insurance
company at the time of the visit.
If your ability to pay is
affected by a prolonged illness, unemployment or other
special circumstances, we will do our best to make
payment arrangements with you.
For your convenience, Southeast
ENT & Facial Plastic Surgery, PC participates
with most health plans. Most insurance plans now require
referrals from the primary care physician before the
office visit, we ask that the patient assist us in
obtaining this information in a timely manner. Any
pre-certification that is required by insurance carriers
for surgery or special procedures will be obtained by
our staff. (For example, balance testing, blood work,
CAT scans, MRI's)
For your convenience, we
are pleased to file insurance claims for you. We charge
patients a $25.00 fee for each disability claim form we
are asked to complete. If we copy records, the charges
are based upon federal guidelines |
