Patient Information
Welcome to Southeast ENT & Facial Plastic Surgery! If you would like us to bill your insurance company, don’t forget your insurance card and co-pay which must be presented at the time of service. For those who do not have insurance coverage, it is expected that charges be paid in full at the time of each visit. If this is your first visit to this office, please arrive 15 minutes before your scheduled appointment.
If your health insurance plan requires you to have a referral for your visits to a specialist, you must coordinate getting that referral through your primary care physician prior to your appointment. Patients without valid referrals will be rescheduled.

Telephone Calls
Our phones are answered 24 hours a day. Calls received after office hours are answered by an answering service. Calls are returned as quickly as possible.
Prescription Refills
Prescriptions and refills for established patients are routinely issued Monday through Friday between the hours of 9:00am and 5:00pm by calling Southeast ENT & Facial Plastic Surgery at (912) 330-5030 or by having the pharmacy fax the request to
(912) 330-5033.
Billing and Insurance Information
Professional Fees
Co-payments, unmet deductibles, and payment for services not covered by your health plan are due at the time services are provided. For those who do not have insurance coverage, it is expected that charges be paid in full at the time of each visit. If you have any questions regarding fees or if you need to make payment arrangements, please speak with our Collections Department.

There are many different types of health insurance coverage. Your responsibilities as a patient depend on the contract between you and your insurance company and whether or not Southeast ENT & Facial Plastic Surgery, PC has a contract with your insurance company.
